ACG Hospice Receives the 2026 HCHB Hospice Quality Award

ACG Hospice, part of the Agape Care Group family of hospice providers, was recently awarded the 2026 Homecare Homebase Hospice Quality Award. The award recognizes organizations who achieve a perfect score of 100 in the Hospice Quality Reporting Program (HQRP).

Homecare Homebase (HCHB) presented the award during its annual conference in September. The award celebrates hospice organizations that demonstrate exceptional performance in the quality measures established through the Centers for Medicare & Medicaid Services (CMS) Hospice Quality Reporting Program.

This recognition reflects ACG’s commitment to living our mission and providing the highest quality clinical care to those entrusted to us. Achieving a perfect HQRP score is a significant accomplishment and a testament to the dedication, expertise, and compassion of our care teams.

The Hospice Quality Reporting Program (HQRP) is a national program established by CMS to promote the delivery of high-quality hospice care. The program collects and publicly reports quality data from Medicare-certified hospice providers, helping hospice organizations measure their performance, identify opportunities for improvement, and promote transparency in the quality of care provided to patients and families. HQRP information is also publicly reported through CMS’s Medicare Care Compare resources.

Homecare Homebase is a leading technology provider dedicated to the home-based care industry. Each year, HCHB recognizes outstanding home-based care organizations through its Customer Awards program.

ACG is honored to receive this recognition and grateful to the dedicated clinicians and team members who make exceptional care possible every day. This award reinforces our commitment to our mission of serving with love, providing comfort and support with compassionate care and meaningful experiences, while delivering the highest quality care to every patient and family we serve.
